Jackson Health System to integrate tools from Allscript's CarePort Health with Cerner EHR

Jackson Health System in Miami has extended its care management partnership with Allscripts company CarePort Health, the post-acute outcomes management software provider confirmed July 11.

Under the agreement, Jackson Health System will implement CarePort Health's discharge planning and referral management platforms at six of its Miami-area hospitals and long-term care facilities. CarePort Health, which Allscripts acquired in 2016, assumed management of Allscripts' care management solution and services portfolio in April.

Jackson Health System and CarePort Health also agreed to develop a first-of-its-kind integration between the hospital's Cerner EHR and CarePort Health's care management platform. The integration creates a single sign-on interface between the EHR and CarePort Health's discharge planning application, allowing clinicians to launch the referral workflow directly from the Cerner EHR.

The integration will streamline clinician workflow and provide a more seamless experience for staff, according to Jackson Health System officials.

In order to achieve this goal, we applied a highly-innovative strategy by designing and building the integration between these two disparate and unconnected systems," Michael Garcia, senior vice president and CIO of Jackson Health System, said in CarePort Health's July 11 statement.
