US-based health and community care technology company WellSky has expanded the capabilities of its CarePort suite of solutions for care coordination.

The new capabilities give discharge planners, care managers and care coordinators more information at the point of discharge to help them make better decisions and enable patients to get the best care and support possible as they shift from hospitals to their homes or other care settings.

CarePoint was acquired by WellSky from Allscripts in 2020.

Based in Overland Park, Kansas, WellSky also recently expanded its care network with the acquisition of Curaspan.

This acquisition enabled customers of Curaspan who use CarePort Discharge to benefit from the new wider range of solutions.

The CarePort Connect solutions give providers access to their discharge planning workflows.

Equipped with this access, care providers can gain a deeper understanding of past post-acute care utilisation, which is crucial for them in determining the right amount of care required post-discharge, and then accordingly develop a care plan unique to a patient’s needs.

Furthermore, hospital readmissions can be decreased as patients can be connected to the best care from providers, who will have access to clinical data and encounter histories when patients are at their most vulnerable phase.

CarePort Guide, which is available in CarePort Discharge, enables hospitals and health systems to have access to a patient choice tool that helps patients and their families in choosing post-acute care.

This guide features post-acute quality scores, agency profiles and guides, and underscores high-performing post-acute networks and collaboratives, which are all sharable through text and email.
