CarePort Health Announces Partnership with TechSpring, Baystate Health’s Informatics and Technology Innovation Center
BOSTON – Nov. 14, 2014 – CarePort Health, a digital health company focused on improving post-acute outcomes, announced today its partnership with TechSpring, a healthcare informatics and technology innovation center affiliated with Baystate Health. CarePort began working with Baystate in 2013 and now formally joins the TechSpring accelerator.
At Baystate, CarePort partnered with the case management department to launch a patient-centered tool to help patients and their families make informed decisions about post-acute care. Using the tool, patients can compare post-acute providers on critical information including quality scores, photos, amenities, and clinical capabilities. With CarePort, patients finally have the resource needed to select a provider that will best be able to care for them. The tool has decreased hospital avoidable days, increased patient satisfaction and generated significant cost savings. CarePort is now expanding at Baystate and will be used by the care transitions team to track post-acute care outcomes for different patient populations.
TechSpring, a collaborative initiative based in Springfield’s emerging Innovation District, will match private enterprises with partners and expertise from Baystate to take on some of healthcare’s most difficult challenges. Other industry partners include Cerner, IBM, Premier Inc., Dell, Medecision, and Mainline Information Systems.
