CarePort Health is pleased to announce that Dr. Lissy Hu, CarePort’s founder and CEO is speaking at Allscripts Client Experience event (ACE) in Chicago August 8-10. In addition to her session on partnering with post-acute care providers to build a high quality post-acute network, Dr. Hu is co-presenting with CarePort and Allscripts client Holyoke Medical Center about the value of care coordination tools for managing multiple patient populations including ACO, BPCI, and complex patient populations.
In a morning session on Wednesday, August 8, Dr. Hu will discuss how health systems across the nation are partnering with post-acute care providers to develop preferred provider programs. Such programs drive quality and financial outcomes for success in comprehensive joint replacement, bundled payment, readmission reduction, and accountable care programs. The discussion will focus on best practices for building high value post-acute partnerships and the implications of preferred provider programs for patient choice, the IMPACT Act and how to extend care coordination to patients beyond the hospital.
In the afternoon that same day, Lissy will join Michael Ipekdjian, Director of Transitional Care Management at Holyoke Medical Center, for a discussion on managing multiple patient populations. Their session will demonstrate how tight coordination between multiple levels of care – powered by connected technology platform – helps keep track of patients as they move between inpatient, post-acute and outpatient settings.
