AHN/Physician Partners of Western Pennsylvania’s Practice Transformation initiative implemented CarePort Connect to optimize their transitional care management (TCM) process and better track and manage patients across care settings. With improved interoperability and more comprehensive data, the physician partners TCM program realized:
