Hospital and health systems chose CarePort platform over incumbent solutions throughout 2020
BOSTON — March 3, 2021 — CarePort, powered by WellSky®, a market leader in care transitions, today announced that more than 130 hospitals and health systems selected CarePort Care Management in 2020 over other discharge planning solutions, contributing to CarePort’s double-digit year-over-year client growth. With new health systems including Atrium Health and UMass Memorial Medical Center now using its platform, CarePort has significantly expanded its acute and post-acute footprint across the United States. The company’s increased scale benefits all providers on the CarePort network by enabling more opportunities for referral and care coordination with new providers.
In 2020, 41 of the more than 130 hospitals that CarePort added to its client roster chose CarePort instead of continuing contracts with incumbent care transitions solutions. According to clients, they chose the CarePort platform because of its key differentiators –– including its comprehensive post-acute database and referral network, post-acute analytics, patient choice tools, and closed loop referral capabilities –– all of which are integrated into the electronic health record (EHR).
