Hear from Todd about how UnityPoint at Home and WellSky are working together on the program designed to provide an elevated level of care in the home, and then share your feedback and perspective about what your organization needs to be successful in participating in the SNF-at-home model. This session aims to bring together like-minded leaders interested in sharing their thoughts and feedback. We’ll be joined by a sommelier who will keep things relaxed by leading tastings throughout the 75-minute discussion.
