CarePort® Patient Event Notification Solution Now Available in Epic App Orchard Marketplace

CarePort’s national network of post-acute providers, physicians, and suppliers

BOSTON — January 28, 2021 — CarePort, powered by WellSky®, a market leader in care transitions, today announced that its interoperability solution, CarePort Interop, is now available within the Epic App Orchard marketplace. CarePort Interop satisfies the patient event notification requirement in the Conditions of Participation (CoPs) from the Centers for Medicare & Medicaid Services (CMS), and this end-to-end integration allows providers to comply with the Medicare CoPs by the April 30, 2021 deadline.

Through CarePort’s integration with Epic’s electronic health record (EHR), acute care providers can now access CarePort Interop directly from the patient record, leveraging FHIR APIs to transfer information between the two applications to maintain patient context. Through existing networks and channels, CarePort Interop helps eliminate the need for new workflows and technology, and doesn’t require any onboarding for post-acute providers and physicians.

CarePort Interop leverages CarePort’s national database, which consists of more than 110,000 post-acute providers — including skilled nursing facilities, home health agencies, long-term acute care hospitals, inpatient rehabilitation facilities, and hospices — as well as 850,000 direct-enabled physicians on ONC-certified EHRs and suppliers including durable medical equipment (DME), infusion therapy, or home infusion therapy. More than 30% of hospital discharges to post-acute providers in the United States flow through the CarePort platform every year.

CMS’s patient event notification CoP requires all hospitals with an EHR to send electronic patient notifications for patient admissions, discharges, and transfers to post-acute providers, primary care providers, physicians, and suppliers. Hospitals that do not comply risk future Medicare funding. CarePort Interop is equipped to send real-time electronic patient event notifications to all in-network and out-of-network providers. These providers include post-acute facilities and suppliers that have recently treated the patient or to whom the patient is being discharged, as well as the physicians primarily responsible for the patient’s care — whether an established primary care provider or practice group or other practitioners such as endocrinologists or oncologists.

CarePort Interop is now available in the Epic App Orchard marketplace, and this is the third CarePort application listed in App Orchard.
