Preferred Primary Care Providers (PPCP), the largest independent primary care practice in Western Pennsylvania, struggled to understand, in real time, patients’ emergency department (ED) visits, readmissions, and referral patterns from hospitals and post-acute providers.
To efficiently monitor cross-continuum patient activity, PPCP turned to CarePort Connect, and has since leveraged the platform in several key areas – including active daily management, transitions of care, and complex care management – to support the organization’s global risk business model.
