This collaborative session will feature a robust Q&A and allow attendees the opportunity to share their biggest data gaps.

In this panel discussion, healthcare leaders will come together to share their care coordination strategies and best practices for building a high-performing post-acute network. Leaders from Advocate Health Care, Rush University Medical Center, and University of Chicago will talk ways to improve quality and financial performance, using technology to develop scalable infrastructure, and how to develop new partnerships through the lens of value-based care.

Sherri Aufderheide, DNP-RN, NEA-BC, ACM, CMAC

Executive Director, Inpatient Care Management and Utilization Management (Illinois), Integrated Care Management

Advocate Health Care

Sherri Aufderheide has been a nurse for twenty-four years, with fourteen years of experience in health care leadership. She is currently the Executive Director of Care Management Operations for the Illinois hospitals for Advocate Aurora Health. She has experience in a wide range of health care settings, including hospital and health system leadership, occupational health, and revenue cycle. She has an MSN in Clinical Nurse Leadership from OSF Saint Francis Medical Center College of Nursing and a Doctor of Nursing Practice from Lewis University. She serves on the executive board of the Illinois Chapter of the American Case Management Associate and is certified in Case Management. She also serves on the Continuum of Care Committee for the Illinois Organization of Nurse Leaders, and is board certified as a Nurse Executive. She has presented locally and nationally and published articles on nursing and care coordination topics.

Tom Martin, director of post-acute analytics at CarePort, powered by WellSky, has led several data analytics teams providing insight to healthcare providers trying to improve quality of care for their post-acute care patients. He holds an M.S. in Resources Economics with a concentration in econometrics from the University of Massachusetts Amherst. Tom studies how the ever-changing post-acute care regulatory and payment landscape is impacting care delivery and how acute and post-acute providers can leverage their data to improve patient care.
