Achieving Health Plan Success: The HEDIS FMC Measure
CMS has proposed the addition of new Part C and Part D HEDIS (Healthcare Effectiveness Data and Information Set) measures from the National Committee for Quality Assurance (NCQA) to the Medicare Advantage Star Ratings program for improved care coordination and transitions.
One of these measures is the HEDIS Follow-Up After Emergency Department Visit for People with High-Risk Multiple Chronic Conditions (FMC), which seeks to ensure follow-up within seven days of ED discharge for members 18 years of age and older who have multiple high-risk chronic conditions for improved cross-continuum provider collaboration and to mitigate communication lapses between the ED and outpatient providers.
