CMS has proposed the addition of new Part C and Part D HEDIS (Healthcare Effectiveness Data and Information Set) measures from the National Committee for Quality Assurance (NCQA) to the Medicare Advantage Star Ratings program for improved care coordination and transitions.

One of these measures is the HEDIS Plan All-Cause Readmissions (PCR) measure, which assesses the rate of adult acute inpatient stays that are followed by an unplanned acute readmission for any diagnosis within 30 days of discharge.
