Achieving Health Plan Success: The HEDIS PCR Measure
CMS has proposed the addition of new Part C and Part D HEDIS (Healthcare Effectiveness Data and Information Set) measures from the National Committee for Quality Assurance (NCQA) to the Medicare Advantage Star Ratings program for improved care coordination and transitions.
One of these measures is the HEDIS Plan All-Cause Readmissions (PCR) measure, which assesses the rate of adult acute inpatient stays that are followed by an unplanned acute readmission for any diagnosis within 30 days of discharge.
Though CMS is still finalizing details regarding the addition of this measure to the Medicare Advantage Star Ratings program, it is critical that health plans begin to put the necessary workflows in place. Download our HEDIS PCR resource to learn how CarePort can help your health plan organization.
