Achieving Health Plan Success: The HEDIS TRC Measure

CMS has proposed the addition of new Part C and Part D HEDIS (Healthcare Effectiveness Data and Information Set) measures from the National Committee for Quality Assurance (NCQA) to the Medicare Advantage Star Ratings program for improved care coordination and transitions.

One of these measures is the HEDIS Transitions of Care (TRC) measure, which incorporates time-sensitive elements known to improve care coordination – including admission and discharge processes – to prevent rehospitalizations, emergency department visits and admissions, and other poor health outcomes.

Though CMS is still finalizing details regarding the addition of this measure to the Medicare Advantage Star Ratings program, it is critical that health plans begin to put the necessary workflows in place.
