Memorial Hermann has partnered with CarePort to safely and securely manage patient episodes across the care continuum.
If you are already using CarePort Intake, you can expect referrals from Memorial Hermann Health System to now seamlessly flow into your Intake dashboard. If you need to update any provider profile information, please reach out to intake.support@careporthealth.com.
