Gloria Rey PA-C, MSPH, Director of Post-Acute Care & Population Health, Henry Ford Health, will share her experiences and results leveraging technology solutions to help manage post-acute utilization for the Henry Ford Accountable Care Organization (HFACO), which was created in 2016 based on the Centers for Medicare and Medicaid Services (CMS) Next Generation ACO (NGACO) pilot model.
