Join Misti Meador RN, BSN, MBA, CCM, Assistant Director of Post-Acute Care at UCSF Healthcare, to explore UCSF’s Transitional Care Program. This session is a must-attend for healthcare leaders and practitioners who are looking to bridge gaps in care, streamline workflows, and reduce healthcare disparities. Discover how UCSF has significantly decreased unnecessary ED  utilization and hospitalization while improving patient outcomes and access to care.

1:00 PM: Strategies for building a high-performing post-acute network

In this group discussion, healthcare leaders will come together to share their care coordination strategies and best practices for building a high-performing post-acute network. Leaders from John Muir and Ensign will talk ways to improve quality and financial performance, using technology to develop scalable infrastructure, and how to develop new partnerships through the lens of value-based care.

Misti Meador RN, BSN, MBA, CCM

Assistant Director of Post-Acute Care

Misti Meador is an innovative leader with 12 years of experience in clinical systems development, case management, and transitional care. She holds a Bachelor of Science in Nursing (BSN) from Clemson University, a Master of Business Administration in Health Care Management from the University of Texas, and a Commission for Case Manager Certification (CCMC).

As Assistant Director of Post-Acute Care at UCSF Health, she is responsible for overseeing the UCSF Preferred Provider Network, the UCSF Transitional Care service, and over 25+ vendor service agreements for the UCSF Care Coordination and Patient Transitions department. At UCSF Health, Misti has made a significant impact in reducing unnecessary hospitalizations, cutting costs, and promoting quality outcomes for her patients.

Misti is well respected among her peers and is regularly sought out as a speaker and thought leader in the healthcare community. She continues to share her expertise and experience with other healthcare leaders so they too can solve complex challenges and drive positive change.
