Care Coordination Trends to Watch in 2023

With 2022 in the rearview, we’re looking forward to 2023. The aftermath of the pandemic, combined with the financial climate and an increasing need for technology and data, have dramatically changed the health care landscape. Just like any new year in the care coordination space, 2023 will bring its fair share of challenges, but also opportunities. Here are some of the trends to watch in the coming year and how CarePort®, powered by WellSky® is supporting our clients through it all.

At a high level:

  • Demand for post-acute care is growing and shifting to the home
  • Patients have increased acuity and comorbidity rates
  • Average length of stay in hospitals remains high
  • Staffing shortages continue to be a challenge

How CarePort innovations can help

In the face of staffing shortages, increasing patient acuity, rising demand, and an aging population, it will be critical for organizations to adapt to these challenges in 2023. While the challenges may seem daunting, there’s an opportunity to positively influence patient outcomes and care.

One of the most important ways providers can adapt is to shift toward care management solutions that enable organizations across continuum with the tools to gain visibility, intelligence, and the ability to improve care coordination and patient care. This is critically important in controlling spend, reducing readmissions, coordinating transitions of care, and achieving optimal health outcomes for patients.

Here are some of the ways CarePort supports our clients by driving innovation:

Expanding our network

CarePort expanded its care coordination footprint across the United States in 2022. Care Transition Platform (formerly Curaspan) joined CarePort, doubling our acute network to provide visibility and influence across the entire continuum of care and help improve patient outcomes. Also in 2022, 52 million referrals were sent through CarePort solutions, a 27% increase over 2021.

Physician groups

Physician groups need better collaboration across the care continuum as practices are entering into value-based care programs because they lack meaningful real-time visibility into patient movement. CarePort Ambulatory bridges these gaps in care and connects physicians to a scaled network of acute, post-acute and home-based providers — giving them the much-needed visibility and intelligence to influence care.

Home health providers

HHAs using WellSky Care Insights and CarePort Connect now have access to real-time hospital admission data from CarePort’s robust national network of nearly 2,000 hospitals, providing more visibility into care events occurring outside the home. With this increased insight, home health providers can make more informed decisions to drive better performance in value-based models.

Epic & Cerner clients

CarePort applications provide Epic users with multiple offerings in the Epic Connection Hub that help advance all activities related to transitions of care without having to leave the Epic platform.

CarePort now provides Cerner Acute Case Management clients with an embedded workflow solution and access to CarePort’s nationwide network of providers, which can help transition patients more  efficiently to the next level of care.

Predictive analytics & referral metric visibility

By considering cost, quality, outcomes, and similar patient profiles, new proprietary algorithms in CarePort support hospitals and health systems in making data-driven post-acute care decisions for their patients.

New referral metrics in CarePort Insight provide data on PAC volume, time-to-respond, and acceptance rates to control costs and build better referral relationships with quality post-acute providers.
