To help ensure nursing home safety during the COVID-19 outbreak, CMS has restricted all nursing home visitors, except in certain compassionate cases, effective March 13. These new measures eliminate the possibility of in-person tours and visits at post-acute care facilities for patients and their families who may still need to seek post-acute care during this time. During nursing home lockdowns, CarePort Guide – an interactive tool used by discharge planners, patients, families and care managers to find high-quality post-acute care for a patient’s specific needs – can help facilitate decisions regarding a patient’s next level of care. Using CarePort Guide, patients and their families have access to detailed information on every Medicare-certified post-acute, home health and hospice agency in the country, as well as an interactive guide that includes virtual tours and pictures when available to view and select post-acute providers. All of this is available to patients and families via text or email. We encourage post-acute facilities using CarePort Guide to update their profiles and upload Virtual Tour links to ensure discharge planners, patients and families can learn as much about a post-acute facility as possible – without ever having to step foot in the facility.
During this public health emergency, CarePort can facilitate more streamlined communication between acute and post-acute providers. On behalf of our hospitals and health system customers, CarePort can send fax notifications to post-acute providers in their geographic regions to communicate COVID-19 protocols, best practices and the status of patients undergoing COVID-19 testing. Transparent, real-time communication and knowledge sharing during COVID-19 is critical for acute and post-acute partners.
