AHN/Physician Partners of Western Pennsylvania’s Practice Transformation initiative implemented CarePort Connect to optimize their transitional care management (TCM) process and better track and manage patients across care settings. With improved interoperability and more comprehensive data, the physician partners TCM program realized:
- 49% year-over-year increase in TCM encounters
- 44% year-over-year revenue increase
HM Home and Community Services, the home and community care subsidiary of Highmark Health, provides health care organizations that are accountable for managing populations within home and community settings with customized solutions for the entire episode following hospitalization, achieving higher levels of performance including lower costs, improved health outcomes, and a better patient experience.
