BOSTON, MASS. – MAY 6, 2020 – CarePort Health, a market leader in care transitions software, today announced a comprehensive interoperability solution to help hospitals achieve compliance with new Conditions of Participation (CoPs) mandating electronic notifications as part of the Centers for Medicare & Medicaid Services’ (CMS) Interoperability and Patient Access final rule. While deadlines for compliance have been extended due to COVID-19, the pandemic highlights the importance of interoperability and real-time patient event notifications to better track and monitor patients as they move through the care continuum.
The new CoPs support post-discharge transitions by requiring that hospitals, or an intermediary on the hospital’s behalf – like CarePort – send electronic patient event (ADT) notifications to post-acute providers, primary care physicians and other organizations when a patient is admitted, discharged or transferred. Though CMS has extended the implementation timeline for the ADT notification CoPs, hospitals must still have policies and procedures in place by spring 2021.
