Integration allows Epic customers to increase conversions to preferred providers while preserving patient choice
Boston, MA – November, 13, 2019 – CarePort Health, a pioneer in care coordination and post-acute outcomes management, today announced that CarePort Guide, its post-acute provider database and patient choice application, is now available in the Epic App Orchard marketplace.
CarePort Guide is an interactive tool used by discharge planners, patients, families and care managers to find high-quality post-acute care for a patient’s specific needs. Through the CarePort-maintained, national database of post-acute providers, patients and families have greater visibility into available post-acute quality of care and services, which helps them make informed decisions about their care. Providers can also use CarePort Guide to fulfill regulatory requirements including the Conditions of Participation for discharge planning and the IMPACT Act by sharing quality information during the transition to the post-acute setting.
“Under new discharge planning requirements that are part of the IMPACT Act, it is critical for providers to incorporate quality and resource use measures into the patient-decision making process during hospital discharge,” said Dr. Lissy Hu, CEO and founder of CarePort. “In this evolving landscape, Epic customers can ensure compliance with new regulatory requirements by leveraging CarePort Guide directly through the Epic EHR.”
Available now in the Epic App Orchard marketplace, discharge planners can access CarePort Guide via a link within the Epic care management module. The interactive tool can be searched and filtered based on available post-acute provider information such as quality, preferred provider networks, geography and available clinical services. CarePort Guide also enables patients and families to review post-acute providers at the bedside, or through text message and email.
CarePort’s entire suite of products – CarePort Care Management, CarePort Referral Management, CarePort Guide, CarePort Connect and CarePort Insight – include thousands of providers across the country, increasing value through both a broad geographic network and a deeper network functionally to track patients and outcomes beyond just admissions, discharges and transfers.
