CarePort and GHA Subsidiary Partner to Offer Comprehensive Solutions to Improve Transitions of Care

CarePort partners with Georgia Hospital Health Services to offer care coordination solutions to help smooth care transitions and improve patient outcomes across the state

BOSTON, MA – July 9, 2019 – CarePort Health, a pioneer in care coordination and post-acute outcomes management, today announced a partnership with Georgia Hospital Health Services (GHHS), the shared services subsidiary of Georgia Hospital Association. The collaboration will provide hospitals across the state with a complete suite of solutions to improve care coordination by educating, guiding and monitoring patients moving across the continuum of care.

GHHS is partnering with CarePort to help provide real-time visibility and transparency into transitions of care for hospitals in Georgia. “We are committed to selecting optimal solutions for our hospitals to ensure our patients are receiving the best care possible,” said GHA Senior Vice President of Business Operations, Bill Wylie. “We understand the importance of transparency throughout care transitions and are committed to providing real-time patient data to improve patient outcomes. Our partnership with CarePort is our latest effort to equip providers with the technology they need to provide highly coordinated, quality care.”
